If they have worked and paid their taxes on schedule, they will be eligible for Medicare. Your father-in-law would be eligible right now. Your mother-in-law would be eligible at 65. This is providing that she does not have a disability that would make her eligible for Medicare before the age of 65. Check this out with Social Security at www.ssa.gov

I don't have an answer directly, but I have noticed that AARP is forever sending me information that includes insurance. Since it is a membership organization, they can get group plans that make things pretty reasonable. It's at least a start in your research.
There may also be long-term plans that need to be looked into specifically, but if your real problem is covering the next five years, you need to have some idea of what she can expect to do when she is eligible for Medicare, and look for an insurance company that knows how to interact seamlessly (do the paperwork for you) to collect your benefits most efficiently. The modern thing is you have one card, and they can change things around in the computer according to what you need. You don't need to keep shopping around.

The best plan really depends on the level of care that he needs. If he is in pretty good shape and does not have a history of major health issues (Diabetes, cancer, heart disease etc) then you might be able to find a reasonable plan.

You may want to consider paying out of pocket and find doctors who charge reasonable prices for his care. One of the problems we have with insurance is that people have grown dependent on insurance. You need to get a non-resident life license in CA. The directions for this would be on the CA insurance department website. Usually it's just a certified statement of good standing from your resident state, and a fee.
Your move is right. Protecting your parents under medi-claim, especially if they are over 60 years, is good and the best move.
The obstacles are, most of the companies not providing medi-claim for seniors. If it is there, that would be costly. for your parents, you should select one which doesn't require more informations or documents.
Approach STAR HEALTH INSURANCE the only company dealing with only health insurance in India, providing medi-claim to seniors. the amount shouldn't vary year to year. You can cover your parents till the age of 75 to 85 (please check this).
There are other companies like ICICI lombard or some others have the facility but the premium is more than double compare with star health.

I have been a certified community health worker since 1996. I have worked in both home care and nursing homes. I have noticed that Nursing homes pay more than home care and the hours are more stable than home care.
The home care agencies I worked for could not guarantee Full time hours and I found myself doing mostly 24 hour live in work to make ends meet. This would be a good job to suppliment your income. Some home care agencies are very busy and others are not. If you do come across one that is busy, then you might not have any problems getting fulltime work. However a car is essential in home care.
In the nursing homes I found the hours to be regular and easy to schedul things out side of work. Most Nursing home positions I found were Union positions. If there are any problems while working in a nursing home - you have the union to help you when dealing with management.

Since Medicare kicks in at 65 in the US, you're not going to find any US Health companies that offer individual coverage to a 66 year old except medicare supplement. Not much market for non resident seniors. Perhaps some coverage from a Mexican company?
